Add Custom Fields
- Go to Settings >> Custom Fields
- Click on Add Field
- Select the Field type that matches the type of answer you want
Most commonly used Field Types
- Radio Button - Single Answer
- Checkbox - Multi Answer
- Single Line
- Multi Line
